For those of you taking the test to earn 1.5 professional development hours (PDH's), you must first print out the test. This is NOT an on-line test; it is a mail-in test. Please write your name on the first page then, fill-in the appropriate squares which match the correct answers. Mail the completed test, registration form, and personal or company check for $25.00 made out to:
